Introduction

Here are the general guidelines to adopt in the event of a possible claim happening:

 

  • Immediately notify your Insurer by telephone of the accident / loss and submit a written statement of claim together with full particulars and supporting documentation to Insurers as soon as possible.
  • In the event of theft, fire, motor vehicle accident or fatal accident, lodge a police report immediately.
  • Where applicable, notify the Fire Brigade immediately.
  • When injury is concerned, seek medical treatment from a qualified medical practitioner immediately.
  • Take all reasonable steps to minimize the extent of the loss or damage.
  • Give immediate notice of claim to the party responsible for the loss / damage.
  • For claims received from third parties, do not admit liability or negotiate settlement or repudiate liability without the written consent of your Insurers. Immediately forward all notices of claim and documents to your Insurers unanswered.

 

Documentation required for claiming purposes:

  • Completed Claim Form.
  • Medical Report from the attending Medical Practitioner.
  • Original medical bills (if policy is extended to cover medical expenses).
  • Medical sick leave certificates.
  • Police report if it is a vehicular accident or fatal accident.
  • Wages records / salary slips for the last 3 months prior to the accident or Letter of Appointment (applies to Group Personal Accident).
  • In the event of fatal accident:
    • Death Certificate.
    • Burial Certificate.
    • Post Mortem Report.
    • Letter of Administration / Grant of Probate.
  • All other information and documentary evidence as the Insurer / appointed adjusters may require other than the above.

Documentation required for claiming purposes:

  • Completed Claim Form.
  • Medical Report.
  • Original medical bills.
  • Labour Assessment, if available.
  • Memorandum of Agreement if it is a fatal accident involving foreign worker.
  • Police report if it is a vehicular accident or fatal accident.
  • Wages records / salary vouchers for the last 6 months prior to the accident.
  • In the event of fatal accident:
    • Death Certificate.
    • Burial Certificate.
    • Post Mortem Report.
    • Letter of Administration / Grant of Probate.
  • Furnish all other information and documentary evidence as the Insurer or appointed adjusters may require other than the above.

Documentation required for claiming purposes:

  • Official Claim Bill.
  • Original Policy / Certificate of Insurance.
  • Original Bill of Lading (if shipment by sea) OR Airway Bill (if shipment by air).
  • Copy of Supplier’s Invoice and Packing List.
  • Custom Declaration Form.
  • Inward / Outward Survey Report.
  • Shipping Agent’s Tally Sheets.
  • Correspondence holding the respective parties responsible for the loss and their respective replies.
  • All other information and documentary evidence as the Insurer / appointed adjusters may require other than the above.

Documentation required for claiming purposes:

  • Completed Claim Form.
  • Police report.
  • Purchase invoices / suppliers’ bills.
  • Packing list.
  • Contract of Carriage / Delivery Note / Consignment Note.
  • Correspondence holding the carriers / freight forwarders / or other bailees responsible for the damage / loss.
  • Furnish all other information and documentary evidence as the Insurer / appointed adjuster may require other than the above.

Documentation required for claiming purposes:

  • Completed Claim Form.
  • Police report.
  • Fire Brigade Report.
  • Purchase invoices / receipts / valuation reports in respect of the damaged items / articles.
  • Stock records.
  • Sales reports.
  • Quotations on the estimated cost of repair to the damaged building and or other properties insured,
  • All other information and documentary evidence as the Insurer / appointed adjusters may require other than the above.

Documentation required for claiming purposes:

  • Detailed Statement of Claim.
  • Sales Reports.
  • Audited Accounts including Detailed Profit and Loss Account.
  • Details of Additional Costs Incurred To Mitigate Losses, if any.
  • Details of Costs Savings, if any.
  • Furnish all information and documentary evidence as the Insurer or appointed adjusters may require other than the above.

Documentation required for claiming purposes:

  • Completed Claim Form.
  • Police report in the event of theft or fire.
  • Fire brigade report in the event of a fire.
  • Photographs depicting the damaged items or location of loss e.g. theft. This will be waived if adjuster is appointed.
  • Quotations on the cost of repairs / original purchase invoices / receipts / valuation reports of the stolen articles or damaged properties.
  • All other information and documentary evidence as the Insurer / appointed adjusters may require other than the above.

Documentation required for claiming purposes:

  • Completed Claim Form.
  • Police report, in the event of a theft or fire.
  • Purchase invoices / receipts / valuation reports of the stolen or damaged articles.
  • Photographs of losses or damages of the affected items, if available.
  • Technical report on the cause and extent of damage, in the event of damage.
  • All other information and documentary evidence as the Insurer / appointed adjusters may require other than the above.

Documentation required for claiming purposes:

  • Completed Claim Form.
  • Police report.
  • For Burglary claims, purchase invoices / receipts / valuation reports of the stolen or damaged items. 
  • For Money claims, copies of your accounting or other documentation to establish the exact amount of loss. This includes copies of receipts, payment vouchers, pay-in slips, cheque butts, bank statements etc.
  • Photographs of the stolen items and point of entry, if available.
  • Where applicable, copy of letter holding security company liable for the break-in and their reply.
  • All other information and documentary evidence as the Insurer / appointed adjusters may require other than the above.

Documentation required for claiming purposes:

  • Completed Claim Form.
  • Police report, where applicable.
  • Copy of equipment registration card.
  • Photocopy of driver’s driving license and identity card.
  • Photocopy of hire purchase agreement, where applicable.
  • Quotations on estimated cost of repair / replacement.
  • All other information and documentary evidence as the Insurer / appointed adjusters may require other than the above.

Documentation required for claiming purposes:

  • Completed Claim Form.
  • Police report.
  • Copy of employment and termination letters issued to defalcator.
  • Latest salary slip and a statement of the balance amount due / amount of claim.
  • Documents substantiating the defalcated amount.
  • Internal investigation report.
  • All other information and documentary evidence as the Insurer / appointed adjusters may require other than the above.

Documentation required for claiming purposes:

  • Completed Claim Form.
  • Police report, where appropriate.
  • Internal investigation report on full circumstances and cause of damage / injury suffered.
  • In the event of injuries sustained, medical report and original medical bills upon receipt.
  • In the event of death, death certificate, post-mortem report and burial permit.
  • In the event of property damage, photographic evidence and quotations on estimated cost of repairs.
  • All other information and documentary evidence as the Insurer / appointed adjusters may require other than the above.

Documentation required for claiming purposes:

  • Completed Claim Form.
  • Police report, where applicable.
  • Photographs depicting the damaged glass.
  • Original repair bill / replacement bill.

Documentation required for claiming purposes:

  • Completed Claim Form.
  • Copy of summon / letter of demand filled against Insured.
  • All relevant documents and information as required by adjusters / solicitors.

Documentation required for claiming purposes:

  • Completed Claim Form.
  • Medical Report in the case of injuries.
  • Death Certificate / Burial Certificate / Post-Mortem Report in fatal cases.
  • Police Report, where applicable.
  • Furnish all information and documentary evidence as the Insurers or solicitors may require other than the above.

Documentation required for claiming purposes:

  • Completed Claim Form.
  • Police Report, if lodged.
  • Copy of letter of award.
  • Repair / replacement quotation.
  • Delivery order of item to site.
